USCIS Document Checklist for Green Card Applications

Applying for a green card in the USCIS is a big step toward living and working permanently in the country. Whether you are applying through family, employment, or another path, you need to submit the correct documents to USCIS (U.S. Citizenship and Immigration Services).

Form I-485 – Application to Register Permanent Residence

This is the main form you must file to apply for a green card from inside the U.S. It is used to adjust your immigration status to that of a lawful permanent resident.

  • Make sure to complete all sections
  • Sign and date the form
  • Include the filing fee (or request a fee waiver if eligible)

Passport-Style Photos

You need to submit 2 recent, color passport photos.

Requirements:

  • 2 x 2 inches
  • Taken within the last 6 months
  • White background
  • Neutral facial expression

Government-Issued ID with Photo

This can be your:

  • Valid passport (preferred)
  • National ID
  • Driver’s license

Make sure the ID shows your photo, name, and date of birth clearly.

Birth Certificate

You must submit a copy of your birth certificate with your full name, date, and place of birth.

If it is not in English, add a certified English translation.

Copy of Passport Page with Non-Immigrant Visa

If you entered the U.S. with a visa (like a student or tourist visa), you need to include:

  • A copy of the visa page
  • The entry stamp showing your arrival date

I-94 Arrival/Departure Record

This shows your official arrival to the U.S. You can get it online from the CBP website (https://i94.cbp.dhs.gov).

Make sure it shows:

  • Your arrival date
  • Your visa type
  • Your current immigration status

Proof of Lawful Entry

If you entered the U.S. lawfully (with inspection), submit evidence like:

  • CBP entry stamp in passport
  • Boarding pass
  • I-94 form

This is very important for adjustment of status applications.

Form I-693 – Medical Examination Report

This form is completed by a USCIS Login-approved doctor. It confirms you meet health requirements.

The doctor will check:

  • Vaccination records
  • Communicable diseases
  • General physical health

Submit this in a sealed envelope provided by the doctor.

Form I-864 – Affidavit of Support

This is required for family-based applications. The sponsor (usually a U.S. citizen or green card holder) fills it out to show they can financially support you.

Documents to include:

  • Latest tax returns
  • W-2 or 1099
  • Proof of income (pay stubs, employment letter)

Proof of Your Relationship

For example:

  • Marriage certificate (for spouse)
  • Birth certificate (for parent/child)
  • Photos, chats, joint bank accounts (to prove marriage is real)

Always include authentic, clear, and relevant proof.

Form I-130 – Petition for Alien Relative

This form is submitted by the sponsor (usually a U.S. citizen or LPR). It starts the green card process for relatives.

Include:

  • Copy of petitioner’s green card or citizenship certificate
  • Proof of relationship

Employment Authorization Document

You can apply for a work permit while your green card is processing.

  • Submit Form I-765 if you want to work in the U.S.
  • No fee if filed with Form I-485

Advance Parole

This form allows you to travel outside the U.S. while your green card is pending.

Do not leave the U.S. without this if your green card is not yet approved!
